Our staff is specially trained to assist you with scheduling appointments and answering any questions you may have. Our goal is to help you maximize any insurance benefits, assist you in filing your dental insurance as a courtesy, and make any remaining balance affordable.
We ask that you carefully review your own policy. Please be aware that your insurance may have frequency limitations, restrictions and maximums benefits allowances. Please remember to bring your insurance card with you to your appointment.
In order to provide you with the highest quality dental care we will provide you with the estimated fees for your dental visit. We ask all patients, guardians or parents to be prepared to pay your estimated patient portion at each visit.
Please print and fill out your new patient forms (to the left), and bring them to your scheduled dental visit.

• We accept visa and master cards, cash or checks at the time of service
• 5% Senior Citizen discount (65 and over)
• 5% Courtesy adjustment for payment in full by check or cash
• 3% Courtesy adjustment for payments with a credit card
• Preferred provider for United Concordia, Delta, Premera and DeCare
